West Plains, MO. – Ozarks Healthcare will offer free athletic physicals for student athletes entering grades 7 – 12 in the 2023 – 2024 school year from 4 to 7 p.m. on Tuesday, May 23, at Ozarks Healthcare Orthopedics and Spine located on the second floor of Ozarks Healthcare’s main entrance (Medical Office Building) at 1100 Kentucky Ave. in West Plains. Local physicians, family nurse practitioners, and licensed practical nurses (LPNs) will conduct the physicals.
Student athletes will check in on the second floor of the main entrance. Once checked in, students will be directed to the appropriate location to complete physicals.
Free athletic physicals will also be offered at all of Ozarks Healthcare’s clinics surrounding West Plains for area students by appointment effective immediately through the remainder of the 2022 – 2023 school year. To make an appointment for your child’s physical, simply call any of Ozarks Healthcare’s clinics in Alton, Gainesville, Mountain Grove, Mountain View, Thayer, Winona, and Mammoth Spring, Arkansas. All students receiving a physical are required to submit the signed consent/medical history form linked at https://www.ozarkshealthcare.com/about-us/for-community/ and provide current insurance information from a parent or legal guardian at the time of physicals.
